Provides comprehensive services to individuals with developmental disabilities, mental retardation, autism, and other pervasive developmental disorders. Individuals are assigned professional service coordinators who are responsible for clients' Individualized Support Agreements, and for assisting them in achieving success in the community. Services include personal support and case management; specialized residential programs; psychiatric and medical care; vocational and skill acquisition training; family support services; and support services to individuals in nursing homes. Provides a spectrum of supported living arrangements to meet the needs of each person. Clinical services for individuals include provision of a psychiatrist to provide evaluation, medication checks, and after-hours consultation on medication issues. Also provides care coordination services. Also includes a text line for individuals who prefer not to call.

Community Care Network - Rutland Mental Health Services (CCN) is the designated community mental health agency contracted by the Vermont Department of Mental Health to serve the residents of Rutland County.It offers a range of programs providing treatment for behavioral health, developmental disabilities, substance use & recovery, as well as residential services. CCN provides acute and long-term behavioral health care services to people who experience a wide range of behavioral and mental health challenges.
