TRANSITIONAL LIVING PROGRAM | NORTHEAST KINGDOM YOUTH SERVICES

Provides essential support services that youth need to make a successful transition to independent living, Services include weekly case management and individualized service planning, life skills coaching/education, assistance with securing safe and stable housing, and assistance with securing mental health and substance abuse treatment, as well as assistance with legal issues. Offers educational strategies and practices, including special instructional methods and materials, learning activities and diagnostic and assessment procedures which encourage children and adolescents to maintain an acceptable grade point average, avoid excessive absenteeism or disruptive behaviors which put them at risk for suspension or expulsion and remain in school through completion of their elementary and secondary education.
